Closing the care loop

Project Overview

More than 50% (130 million) of US adults live with multiple chronic conditions [1]. Heart disease, diabetes, Alzheimer's -- all require coordinating between multiple care teams, medications, symptom tracking -- and often fall onto the shoulders of overburdened caregivers. Avina pieces these points of contact with missing information, surfacing trends (”care signals”) and changes with AI so care isn’t delated, and leads to insurance reimbursed to staff effort. (from site; “Patients’ needs change between visits, but your team often doesn’t know until a caregiver calls. Staff then piece together what changed and manually find resources one family at a time, while families are left to manage the paperwork and follow-up themselves, delaying care and leaving hours of staff effort unreimbursed.”)

I was tasked with creating the 0 -> 1 mobile interface for caregivers that consolidates all care signals, as well as defining the brand’s visual identity (at time of joining, the company only had an information website with undefined visual elements).

Context

What Avina is

Patients’ needs change between visits, but your team often doesn’t know until a caregiver calls. Staff then piece together what changed and manually find resources one family at a time, while families are left to manage the paperwork and follow-up themselves, delaying care and leaving hours of staff effort unreimbursed.

Design Challenge

Avina didn’t have an interface, much was being done manually as the technology was being developed. So I sat in on user interviews and collaborated with the founder to replicate the personal care process she delivered to each patient. Should include info on why Avina needed an interface (though our product is an AI triaging service with an SMS AI agent, it still needs an interface where users can 1) answer check-in forms and 2) see their patient’s status overall)

Key Deliverables

01 Caregiver Interace

Mobile interface that includes regular intake form and

02 Design System

Visualized accessible, colour-blind friendly interface that consolidates the Care Signals

03 Something

I haven’t thought of something here yet

Research
What the personal care process was like:
1) Regular biweekly check-in’s where (usually) caregiver answers questions corresponding to 1 or 5 Care Signals (Mood, Sleep, Function, Incidents, and Caregiving)
